Services Structure Process for (SDP)

1. Orientation Sessions
Prospective participants must attend a two-part virtual orientation through Zoom.

2. Person-Centered Planning
After completing orientation, the participant works with their circle of support to create a Person-Centered Plan (PCP).

3. Individual Budget Determination
The Regional Center calculates the participant’s individual budget based on the past 12 months of service expenditures and any newly identified service needs.

4. Selection of Service Providers
Participants have the flexibility to choose their own service providers, including individuals or agencies.

5. Servicios de Manejo Financiero (FMS)
Los participantes deben trabajar con un proveedor de Servicios de Manejo Financiero (FMS), quien les brindará asistencia para administrar los fondos.

6. Aprobación e Implementación
Una vez que el PCP y el presupuesto estén finalizados y aprobados por el Centro Regional, los participantes pueden comenzar a implementar su plan y recibir servicios.

7. Proceso de Inscripción
La duración desde la orientación inicial hasta la participación completa puede variar según las circunstancias individuales, la complejidad del PCP y el tiempo requerido para establecer los apoyos y servicios necesarios.
