Self-Determination Program (SDP)  Pre-Enrollment

This guide will help individuals move from pre-enrollment to full participation in California's

Self-Determination Program (SDP) under the Regional Center system.

1: Learn About the Self-Determination Program​

  • You cannot live in a licensed long-term care facility unless you are transitioning to a community setting.

2: Pre-Enrollment Phase

  • Contact your Regional Center’s SDP liaison and request to enroll formally.

  • The IF assists with Person-Centered Planning (PCP), budget creation, and service selection.

​​3: Develop a Person-Centered Plan (PCP)​

  • You can create this independently, with family, or with an Independent Facilitator.

  • The PCP will help structure your Spending Plan and Individual Budget.

4: What is SDS?

  • Self-Directed Support is funding provided by the Regional Center to help individuals transition into the

       Self-Determination Program (SDP).

5: Who Qualify for Self-Directed Support:

  • Individuals who are transitioning into SDP through a Regional Center.
