Person -Centered Planning (PCP) Process
How the PCP Process Works:
​This process is guided by the individual, with input from their Circle of Support, an Independent Facilitator (IF), and the Regional Center:

1. Preparing for the PCP Meeting:
Before the meeting, we establish key details to ensure a smooth and effective discussion:​

Circle of Support: Identifying key individuals who will participate.

Accommodations: Ensuring accessibility and support for all participants.

Cultural & Language Considerations: Addressing any language or cultural barriers to ensure clear communication.

2. Understanding Your Needs and Goals:​
During the PCP meeting, the individual leads the discussion, sharing their:

Dreams, Goals, and Aspirations

Preferred living, work, and community participation options​​​​​​​​
3. Developing Your Person-Centered Plan;
​Once the Person-Centered Plan is finalized, the Independent Facilitator presents it to the Regional Center Coordinator for budget review.

Budget Determination – Calculates an individual budget based on a 12-month cost history, considering new needs.

Plan Adjustments – The IPP team may modify the budget based on changing circumstances or unmet needs.

Approval & Implementation – Once approved, the plan is ready for implementation.

4. Spending Plan:

The Spending Plan ensures that all necessary services are properly identified and funded,

It is a key component of the Self-Determination Program (SDP).

5. Components of the Spending Plan:​

Allocates Funds Appropriately

Supports Goal Achievement
6. The Financial Management Service (FMS);

Managing funds and ensuring payments to service providers.

Tracking expenses to maintain compliance with the approved budget.
